Notice of Privacy Practices
Effective date: November 7, 2026
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Lehigh Valley Chiropractic, PC (“we,” “our office”) is required by law to protect the privacy of your health information. This notice explains how we may use and share that information, your rights, and our responsibilities. It applies to all care provided at our office, including chiropractic and functional medicine.
How we use and share your health information
We generally use and share your health information for treatment, payment and health care operations. For example:
- Treatment: we may share information with your physician or another provider involved in your care, such as reporting exam findings to the doctor who referred you. We may also look up your prior imaging and records through the electronic systems of local health networks, such as Lehigh Valley Health Network (Jefferson Health) and St. Luke’s, when it helps us care for you.
- Payment: we may send records and bills to your insurance company to be paid for your care.
- Health care operations: we may use your information to run our office and improve care, such as quality reviews, training, and credentialing.
Appointment reminders and health-related services
We may contact you with appointment reminders, including by phone, text or email, and with information about treatment options or other health-related services that may interest you.
Family members and others involved in your care
Unless you object, we may share information with a family member, close friend or other person you identify who is involved in your care or in paying for it, and we may use or share information to notify them of your location or general condition. If you are not present or able to agree, for example in an emergency, we will use our professional judgment about what is in your best interest.
Other uses and disclosures allowed or required by law
We may also use or share your health information without your written permission when the law allows or requires it, including:
- As required by law, including to the U.S. Department of Health and Human Services to show we are complying with privacy law.
- Public health and safety: for example, reporting disease, injury, adverse reactions to products, or suspected abuse, neglect or domestic violence, and to help prevent a serious threat to someone’s health or safety.
- Health oversight: to agencies for audits, investigations, inspections and licensing.
- Lawsuits and legal actions: in response to a court or administrative order, subpoena or other lawful process.
- Law enforcement: for certain law enforcement purposes, such as responding to a valid legal request.
- Coroners, medical examiners, funeral directors and organ donation organizations, as the law allows.
- Research: under limited conditions that protect your privacy.
- Workers’ compensation: to comply with workers’ compensation laws.
- Specialized government functions: such as military, national security and protective services, as the law allows.
Uses that require your written permission
We will not use or share your information for other purposes without your written authorization. In particular, we need your written permission to use your information for marketing or to sell your information. We do not sell health information. If you give us permission, you may take it back in writing at any time, except for actions we have already taken based on it.
Your rights
You have the right to:
- See and get a copy of your health and billing records. Ask us in writing. If we keep your records electronically, you may ask for an electronic copy. We may charge a reasonable, cost-based fee.
- Ask us to correct your records if you believe they are incorrect or incomplete. Ask in writing. We may say no, and if we do we will tell you why in writing within 60 days. You may then file a statement of disagreement.
- Ask for confidential communications, such as contacting you at a different phone number or address. We will agree to reasonable requests.
- Ask us to limit what we use or share. We are not required to agree, except in one case: if you pay for a service or item out of pocket in full, you can ask us not to share that information with your health insurer for payment or operations, and we will agree unless the law requires us to share it.
- Get a list of those with whom we have shared your information (an “accounting of disclosures”) for up to six years before the date you ask. The list will not include disclosures for treatment, payment or health care operations, disclosures you asked us to make, and certain others. We will provide one list a year for free.
- Get a paper copy of this notice at any time, even if you agreed to receive it electronically.
- Choose someone to act for you. If you have given someone medical power of attorney, or someone is your legal guardian, that person can exercise your rights.
- Be notified of a breach. We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.
To use any of these rights, contact our Privacy Officer (below).
Our responsibilities
We are required by law to:
- Maintain the privacy and security of your protected health information.
- Give you this notice of our legal duties and privacy practices.
- Notify you following a breach of your unsecured health information.
- Follow the terms of the notice currently in effect.
We may change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available on request, in our office, and on our website.
